Sample Cover Letter For Survey

Dear Member:

AFSCME is committed to making every effort to end sexual harassment. The enclosed survey is an attempt to assess the degree to which sexual harassment is a problem for our membership. Therefore, we are asking you to take a few minutes to complete the questionnaire and return it in the enclosed envelope. Also, enclosed is a fact sheet on sexual harassment.

You need not sign your name. However, if you are presently dealing with a sexual harassment problem, we urge you to contact your union representative.

Sexual harassment is against the law and violates our union contract. Working together, we can stop it.
